If you wish to use mediation threshold checking, you need to use the TGI Medication Order Form v3. The variation associated with the medication and dose amount for Methadone Tablets, Methadone Liquid and Suboxone Films would be the same for either order form.
NOTE: You will use TGI Medication Order Form v2 OR TGI Medication Order Form v3. NOT BOTH.
How the medication threshold checking works:
The order form checks for a previous order with the same medication and if the change in dosage exceeds the min and max thresholds setup in Clinic Options it will force you to complete the reason for changing the dosage section of the form (ONLY if you have exceeded the min or max threshold set in Clinic Options).
Writing a TGI Medication Order Form v3 (No previous Order found for Medication for Patient):
- Click on TGI Medication Order Form v3 from the Available Forms list to open it.

Order Date – Defaults to the date the form was first opened/created.

Start Date – Select the date of which the patient is to start receiving medication (This is the first date of which the patient will appear in the Medication Dispensing Screen).

Discontinue Date – If known, select the date of which the patient’s medication is to discontinue (at midnight of the day selected). If left blank, the order will be continuous and NEVER expire.
- See below for steps on how to discontinue an order.
+X – This is the discontinue date interval set up in Clinic Options 🡪 Dispensing Tab. Click this button to set the discontinue date as the indicated number of days after the start date.
Medication- Select the medication to be dispensed (Methadone HCL Oral Concentrate).
Dose Amount- Enter the maximum amount per dose (EX: 10 mL would be entered as 100 mg).
Dose Unit- Select mg.
Guest Dosing- Flag Yes if patient is receiving dosing at a clinic other than their own, otherwise flag No.
Last Active Order for Medication on Chart- This will say “No previous active/dispensed Order found for Medication for Patient.”
Select the Instructions Tab.
Treatment Instructions (Optional)- Enter any desired Treatment Instructions. This would be used for specific dosing/tapering instructions for the dispensing nurse.
Admin Instructions (Optional)- Enter any desired Admin Instructions. This would be used for administrative purposes such as if the order should be discontinued at some point.
Order Summary- Displays a summary of the information entered on the Order Tab.

Confirm- If the order has been reviewed and is confirmed to be correct flag Confirm.

Cancel- If information has not been reviewed and has not been confirmed to be correct, Cancel can be flagged, and the order saved to be edited at another time.
Sign- A Nurse’s signature is required to sign/lock the form, unless marked N/A which would require a Doctor’s signature to sign/lock the form.
Save- Click Save to create the Order Form (You can now write the accompanying schedule).
Writing a TGI Medication Order Form v3 (New dosage exceeds threshold set in Clinic Options):
NOTE: A previous or discontinued order has to have existed in order for the dose amount to change and thus exceed the threshold set in Clinic Options.
- Click on (new) TGI Medication Order Form v3 from the Selected Chart list to open it.

Last Active Order for Medication on Chart- This will display the previous order and once you select the new dosage (exceeding the set threshold) you will see the following warning: “WARNING: Dose Amount beyond Threshold! New dose: 60mg Old Dose: 50mg”. You must now complete steps 11-13.
The reason for changing the dosage- Select a reason for changing the dosage (pulls from Dosage Change Reason lookup table).

If Other, specify- If the reason for changing the dosage is not available in The reason for changing the dosage drop-down flag Other in that drop-down tpe your reason.
I confirm I have done the following- Flag the boxes associated with the task(s) that have been completed (pulls from Dosage Change Check-list lookup table).
I attest that I HAVE completed the above listed items- Flag Yes to attest to the completed task(s).
